This document is a report by the National Security Agency's (NSA) Signals Intelligence Directorate (SID) Office of Oversight and Compliance. The report assesses instances of "incidents" (defined as "unauthorized collection, processing, access, and retention of data") contrary to the NSA's own internal policies and legal restrictions such as Executive Order 12333 and FISA. Detailed descriptions of specific incidents are included in the annexes to the main report.
